When calculating pediatric doses for older or heavier children, always check that the calculated weight-based dose does not exceed the maximum recommended adult dose.

Ideal Body Weight Equations

The Devine formula is the most widely used for IBW: 50 kg (men) or 45.5 kg (women) + 2.3 kg for every inch over 5 feet.
